This agreement outlines the terms, expectations, and scope of our professional coaching relationship. Please read it carefully before signing. By purchasing a coaching package or session and electronically signing this agreement, you acknowledge that you have read, understood, and agree to the terms below. Coach: Amy Burchett, Certified Life Coach Client: The individual purchasing coaching services through Live Above Coaching Coaching Engagement This agreement applies to all coaching services purchased, including individual sessions and coaching packages, beginning on the date of purchase and continuing for the duration of the selected program. 1. Scope of Coaching Services Life coaching is a professional, collaborative partnership designed to support personal growth, mindset renewal, goal achievement, and life transition support. Coaching may include exploration of values, beliefs, habits, emotional awareness, faith, motivation, and action steps. Coaching is forward-focused and centered on clarity, growth, and intentional living. Coaching is not therapy. Coaching does not diagnose, treat, or cure mental health conditions and is not a substitute for psychotherapy, counseling, medical care, legal advice, or financial services. If it becomes clear that your needs fall outside the scope of coaching, you may be referred to a licensed professional. 2. Coach & Client Responsibilities The Coach agrees to: Provide a supportive, respectful, and confidential coaching environment Use professional training, experience, and intuition to facilitate growth and insight The Client agrees to: Take full responsibility for their choices, actions, and results. Actively participate in the coaching process. Understand that results are not guaranteed and depend on personal effort and commitment. Coaching is a collaborative process; however, the Client remains fully responsible for decisions made during and after coaching. 3. Confidentiality & Legal Exceptions All information shared during coaching sessions is confidential and will not be disclosed without written consent, except when required by law, including: - Suspected abuse or neglect of a child, elder, or dependent adult - Imminent risk of harm to self or others - Court order or legal obligation Electronic communication (email, text, video conferencing, online platforms) will be handled with care but cannot be guaranteed to be completely secure. 4. Nature of the Coaching Relationship Life coaching is not a replacement for licensed mental health care, medical treatment, legal counsel, or financial advice. If you are working with a therapist, physician, or other professional, you are responsible for ensuring coaching complements your overall care. The Coach reserves the right to pause or terminate coaching services if it becomes clear that coaching is not appropriate or effective for your needs. 5. Scheduling & Cancellation Policy - Sessions are scheduled through Simplero. - Appointments are reserved specifically for you. - A minimum of 24 hours’ notice is required to cancel or reschedule a session. - Sessions canceled with less than 24 hours’ notice or missed sessions will be charged in full, except in the case of a documented emergency. - Repeated no-shows or late cancellations may result in termination of the coaching relationship. 6. Payment & Packages - Coaching services are billed per session or as part of a prepaid package, as selected at checkout - Payment is due at the time of purchase unless otherwise agreed in writing - Accepted payment methods include credit card and any options offered through Simplero - All prices, package details, and session quantities are defined on the sales page at the time of purchase. 7. Termination & Refund Policy Either party may end the coaching relationship at any time. A final session for closure is recommended when possible. Refunds for unused sessions in prepaid packages are issued at the Coach’s discretion, minus sessions already used and any applicable administrative fees. No refunds are provided for completed sessions. 8. Acknowledgment & Agreement By electronically signing this agreement, you acknowledge that: - You understand the nature and limitations of life coaching - You accept full responsibility for your choices and outcomes - You have read, understood, and agree to all terms of this agreement This agreement is effective upon purchase and electronic signature.
